Central Auditory Processing Disorder (CAPD)

Central Auditory Processing Disorder (CAPD) is a neurobiologic deficit in the central auditory nervous system that affects fundamental auditory perception mechanisms. Individuals with CAPD typically hear normally but have trouble processing what they hear, especially recognizing slight differences in similar-sounding words amidst background noise.

CAPD affects approximately 2–7% of school-aged children and can occur in adults due to various causes including genetic factors, head trauma, neurological disorders, chronic ear infections, or neuromaturational delay. Common symptoms include difficulty localizing sound, understanding rapid speech, following multi-step directions, maintaining attention, learning foreign languages, and detecting changes in speech rhythm or intonation.

Diagnosis requires behavioral and electrophysiologic audiologic tests sensitive to central auditory nervous system dysfunction, typically performed by an audiologist when a child scores two standard deviations below the mean on multiple tests. CAPD is usually diagnosed around age 6 years.

Intervention strategies include auditory training, direct skills training, compensatory strategies, and environmental modifications tailored to specific auditory processing deficits. While CAPD has no cure, these treatments help individuals hear and communicate better. CAPD often coexists with ADHD, language disorders, and learning difficulties, requiring multidisciplinary assessment and collaborative care.
